What We’re Looking For
Trillium Health Resources has a career opening for an IDD Care Manager to join our Integrated Care Management team. As a member of the team, you’ll be responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, or complex medical/physical health need. This position is located and operates out of Trilliums Southern Region. As an IDD Care Manager, you’ll be required to use a dynamic, proactive approach to assessment, monitoring and complex coordination of care, to ensure quality supports and consistent adherence to waiver requirements. You’ll be working directly in your community to help members live their best lives.

On a typical day, you might:
·       Utilize person centered planning methods/strategies to gather information, to get to know the members supported and to adequately address specific needs of the individual.
·       Provide education and support to members in learning about and exercising rights and responsibilities.
·       Monitor per established timeframes to ensure quality care, health/safety of the individual, as well as the continued appropriateness of services as needed.
·       Educate members/families on details surrounding the member’s service budget.

Qualifications:
Education: Bachelor’s degree in health, psychology, sociology, social work, nursing or other relevant human services area, or licensure as a registered nurse (RN)
Experience: Two (2) years’ experience working directly with individuals with I/DD or TBI
License/Certification: None identified
Must have a valid driver’s license
Must reside in NC in Southern Region
